摘 要:对3448名35~54岁中年男性和女性的基线资料分析表明,用力肺活量(FVC)和第一秒用力呼气量(FEV1)均与收缩压和舒张压负相关(r=-0.10~-0.17,P<0.0001)。此关系在对呼吸功能进行身高调整后仍存在(r=-0.11~-0.18,P<0.0001),进一步调整年龄后则明显减弱。其中FEV1与血压的负相关不再具显著性,FVC与收缩压和舒张压的负相关仍具显著性(r=-0.06~-0.10,P<0.05)。对其中2083名基线时血压正常者4年后的复查资料分析表明:基线呼吸功能较低者,高血压发病率较高。但此关系多不具有统计学显著性。调整年龄、基线血压及其它协变量的多元Logistic回归分析得出相同结果。上述结果表明,FVC与血压之间存在负关联,其在中国人群中与高血压发病的关系尚有待进一步研究。Abstract Objective:To study whether there is also an inverse association between lung function and incidence of hypertension in Chinese population.Subjects: As part of a PRC-USA cardiopulmonary epidemiology study,two populations in Beijing were investigated,including a total of 3 448 men and women,age 35-54 at entry.Methods:Participants were studied first at baseline and then 4 years later.Cross-sectional relationship between lung function and blood pressure for baseline and longitudinal relationship between baseline lung function and subsequent incidence of hypertension were analyzed.Results:baseline lung function varied inversely with baseline systolic and diastolic blood pressure.This association stayed after height-standardization in forced vital capacity (FVC)and one-second forced expiratory volume (correlations-0.10 to -0.17,P<0.0001).After adjustment for age,this association was weaker and was still statistically significant for FVC(correlations-06 to-0. 10).Follow-up (4 years) of 2 083initially normotensive subjects showed that higher initial levels of lung function were associated with a lower incidence of hypertension(defined as systolic blood pressure≥140 mmHg or diastolic blood pressure≥90 mmHgor currently using anti-hypertensive medications).These associations were generally statistically insignificant. Logistic regression analyses adjusting for age,baseline systolic blood pressure, and other covariates showed same findings.Conclusion:These results suggest that the inverse relationship between lung function and blood pressure is attenuated by adjustment for age. The predictive role of lung function on incidence of hypertension among Chinese needs further investigation.1 mmHg=0.133 kpa
