摘 要:采用转铁蛋白受体(TfR)放射配体结合分析法及红细胞内外铁染色检查,对41例中期孕(16~28孕周)胎肝幼红细胞TfR及红细胞内外铁进行了研究。结果发现:①在孕母铁状况正常情况下,胎肝有一定数量的细胞内外铁存在,但明显较成人为少。随着孕母缺铁性贫血(IDA)程度的加重,胎肝中红细胞外铁贮存减少,中度IDA时几乎全部消失(等级相关r=一0.55,P=0.0061)。胎肝红细胞内铁的变化与胎肝红细胞外铁变化趋势相同(r分别一0.48及一0.55,P分别为0.009及0.006)。②对胎肝幼红细胞的TfR的研究发现,隐性缺铁的孕母,其胎儿肝脏TfR与正常无明显差异,但IDA母亲的上述TfR位点数明显高于正常组(P均<0.05),提示胎儿因铁代谢受到影响而出现了相应代偿性变化。上述结果提示,中期孕母亲患缺铁性贫血对胎儿肝脏铁代谢有直接的不良影响。rythroblast transferrin receptor ( TfR ),hemosiderin and sideroblast counts in 41 fetal livers inthe second trimester of pregnancy were studied by radioreagent assay and iron staining. It was found that,1. Certain amounts of iron were existed in the fetal liv-er when the maternal iron status was normal. Bothhemosiderin and sideroblast counts in fetal livers weredecreased when the mothers became anemia, and theywere correlated with the degree of maternal iron defi-ciency(ID). 2. The erythroblast TfR levels (sites/cell)of the fetal livers were not significantly different fromthose of the normal countparts when the mothers hadID with no anemia, but they were elevated when themothers had IDA. The results indicated that in thesecond trimester of pregrancy, the maternal IDA haddirect adverse effect on the fetal liver iron metabolism.
