尿微量转铁蛋白酶免疫测定法及其临床应用

摘　　要：为了及时对糖尿病肾病进行早期诊断，建立了一种敏感、简便、可靠的尿微量转铁蛋白（ＴＲＦ）酶免疫测定法。工作范围０．５～１２８μｇ／Ｌ，检出限０．２６μｇ／Ｌ，批内变异系数（ＣＶ）６．３％，批间ＣＶ８．８％，回收率９７．３％。８０例正常健康人的尿ＴＲＦ值以μｇ／ｍｍｏｌＣｒ表示，其均数、几何均数、中位数和９５％位数分别为４４．９、３６．１、３２．３和１２７．５。正常人、糖尿病和肾小球疾病的尿ＴＲＦ和尿白蛋白（Ａ１ｂ）之间均呈现显著正相关性（ｒ＝０．６０～０．８６，Ｐ＜０．０１～０．００１）；疾病组中，尿ＴＲＦ增高幅度较尿Ａｌｂ增高幅度大。尿ＴＲＦ是一种肾小球滤过功能不全的敏感指标。e have depveloped an enzyme immunoassay for microtransferrinuria,This assay was simple,sen-sitive and reliable. The working rang was 0.5 to 128μg/ml, the detection limit 0. 26μg/Land within-and between-assay CV were 6.3%and 8.8%respectively. The mean,geometric mean and median oftransferrinuria in a group of 80 healthy subjects were 44.9, 36.1 and 32.3μg/mmol Cr respectively and the 95th percentile was 127.5μg/mmol Cr. Correlation coefficients between transferrinuria and albumin-uria in healthy subjects,diabetics and glomerulopathy were 0. 6 to 0. 86.The urinary excretion of trans-ferrin was more elevated than that of Albumin. Excretion of transferrin in urine is a sensitive index ofthe glomerular dysfunction.
