中西医结合治疗先天性髋脱位方法的选择与应用

摘　　要：采用７种手术方法治疗先天性髋脱位５４９例（７３９髋），得到６个月～１７年随访的４９６例（５９７髋）中，优７５．２１％，良１３．２６％，可６．９７％，差４．６９％，优良率８８．４７％。临床观察表明，手术治疗先天性髋脱位，年龄越小效果越好，最佳年龄为３～５岁。术式的选择需根据患儿的年龄、脱位程度、髋臼及股骨头发育情况而定，不能只采用某一种方法。重点讨论了７种手术的适应症和术中应注意的问题，阐述了术前牵引和术后外固定的重要性。Five hundred and forty-nine caese (739 hips) of congenital hip dislocation (CHD) were treated by 7 surgical methods. The 0. 5-17-year follow-up of 496cases (591 hips) showed excellent, good, fair and poor rate of 75. 21%, 13.36%, 6. 97% and 4. 69%, respectively. The excellent-good rate reached 88.47%. The clinical observation suggested that in CHD treatment, the younger patients are treated,the better results can be obtained;the optimal age of treatment is 3-5 years. Surgical methods should be selected according to the age ,severity of the dislocation and the development of acetabulus and femoral head of the patient. No single method should be selcted for all patients. The indications and points for attention of the seven surgical methods and the importance of pre-operative traction and post-operative external fixation are discussed in the paper.
