女性性征异常患者外周血白细胞性激素受体改变及意义

摘　　要：应用放射配体结合法检测先天性肾上腺皮质增生１３例，先天性阴道闭锁１３例，多囊卵巢综合征１６例及男性化卵巢肿瘤１３例的外周血白细胞雌激素受体（ＥＲ）及雄激素受体（ＡＲ）。同时应用放射免疫分析检测血浆睾酮（Ｔ）及雌二醇（Ｅ２）。先天性肾上腺皮质增生患者血Ｅ２正常，但ＥＲ降低，血Ｔ及白细胞ＡＲ均增高；先天性阴道闭锁患者血Ｅ２及ＥＲ均降低；多囊卵巢综合征患者血Ｅ２、Ｔ及ＡＲ均升高；男性化卵巢肿瘤血Ｔ、ＡＲ均明显升高。以上结果表明血Ｅ２和白细胞ＥＲ降低及血Ｔ和白细胞ＡＲ增高，在女性性征异常的发生及发展中起作用。Specific binding capacity of androgen receptor (AR) and estrogen receptor (ER) on the peripheral leukocytes was detected by radioligand binding assay and plasmal levels of testosterone (T) and estrogen (E2) by RIA in patients with sex differentiation disorders including thirteen congenital adrenal cortical hyperplasia,thirteen congenital atresia vaginalis, sixteen polycystic ovary and thirteen Sertoli-Leydig cell tumor to determine the effect of sex hormones and their receptors on pseudohermaphroditism.The plasma level of E2 was normal and ER lowered, while T level and AR were increased in congenital adrenal cortical hyperlpasia. Both E2 level and ER were decreased in congenital atresia vaginalis. The plasma levels of E2 and T and binding capacity of AR were elevated in polycystic ovary.The T level and AR were markedly increased in Sertoli-Leydig cell tumor. The findings indicate that both the lowered E2 level and ER binding capacity,and the elevated T level and AR binding capacity play a role in the development of female sex differentiation disorders.
