喉全切除后发声重建的探讨

摘　　要：本文将活瓣性气管食管分流术１２８例的临床资料加以分析讨论。结果除失访者１３例外，１１５例中９１例（７９．１％）发声成功，占本术式的７１．１％。不能发声的２４例（２０．９％）中，６例因局部感染后瘘孔扩大，引起误咽而缝合瘘孔，改练食管发声。另外１８例中，７例患者曾练习发声，但未成功。其原因考虑为：①环咽肌和咽缩肌痉挛；②术中下咽粘膜和食管粘膜切除过多。术后咽腔和食管腔狭窄。另１１例属高龄和体弱患者，无发声欲望或对发声失去信心，放弃发声努力。Abstract The vocal rehabilitation is 115 cases of laryngectomized patients with a recheoe-sophageal shunt were performed from 1 986 to 1 993. The results showed that 9 1 cases could speak understandablly on the aveage 23. 1 days after the operation. Nine of them could speak after dilating the narrowed tracheoesophageal fistula. 24 cases failed to speak because of local infection,muscular spasm of pharynx, stenosis of the esophageal or phryngeal cavity ,the bad health condition,aging and so on. In this artical, we also discussed the procedure of the operation and some other technical problems.
