肝静脉血流多普勒频谱变化评估右心功能  

ASSESSING RIGHT- HEART FUNCTION BY CHANGES OF HEPATIC VEIN FLOW DOPPLER SPECTRUM

摘　　要：应用彩色多普勒超声心动图观察了３０例正常人和２８例心脏病病人的肝静脉血流（ＨＶＦ）频谱及其与右心功能间的关系。结果表明：病例组ＨＶＦ频谱变化与右心功能障碍有关。其中ａＶｍ、ａＶＩ、明显增大，ｄＶｍ、ｄＶＩ显著降低，符合右室舒张早期松弛功能减退；ｖ波持续时间（ＶＴ）延长与右室等容舒张期（ＲＩＶＲＴ）较长一致；ｓＶｍ、ｓＶＩ减低可能与右室收缩功能减退、三尖瓣返流等因素有关。综上所述，本文认为：ＨＶＦ频谱变化可反映右心功能状态。The hepatic vein flow(HVF)velocity wave-forms and some indexes of right-heart function were examined in 30 normal subjects and 28 patients of congenital heart di-sease by color Doppler echocardiography.The results showed that the changes of HVF spec-trum in patients group was concerned with impaired right- heart function.Increaxed aVm、aVI (P<0.001) and decreased dVm、dVI(P<0.01) of HVF spcctrum suggested abnormal rightventricular relaxation,prolonged VT (P<0.05)was in accord with prolonged RIVRT (P<0.05),decreased sVm、sVI(P<0.01)was concerned with impaired right ventricular systolicfunction、tricuspid regurgitation、etc. We concluded that the changes of HVF spectrum couldbe a new useful way in evaluating right-heart function.
