气管及食道异物取出术中高频喷射通气供氧

摘　　要：本文报告全身麻醉下进行气管异物和食道异物取出术应用高频喷射通气（ＨＦＪＶ）２９例结果。气管异物２４例，食道异物５例；采用国产ＫＲ－Ⅱ型呼吸机，频率为６０次／ｍｉｎ，驱动压为０．６ｋｇ／ｃｍ￣２或０．８ｋｇ／ｃｍ￣２，呼吸比１：１．５～２为预选参数。结果，全部手术顺利，术中无任何意外及并发症出现，术前有Ⅲ°呼吸困难的患儿在ＨＦＪＶ供氧后２～３ｍｉｎ即可缓解，因支气管镜检查刺激喉气管粘膜发生的喉痉挛及呼吸暂停在给予ＨＦＪＶ供氧后很快恢复正常，５例食道异物取出过程中未出现窒息和缺氧表现。作者认为，全麻状态下进行呼吸道异物和食道异物取出术，ＨＦＪＶ可改善术前缺氧状态，减轻窒息，保证术中供氧，减少手术并发症，增加手术的安全性。This paper reported the results of 29 cases that high frequency jet ventilation(HFJV)was used in the operations of trachea and esophagus foreign body under generalanesthesia. The 24 cases were trachea foreign body and the other 5 cases were esophagus fo-reign body.The model of breathe machine was KR-Ⅱmade in China. The working frequencywas 60 times/min.The drived press was 0.6 kg/cm￣2 of 0.8 kg/cm￣2.The parameter was 1:1.5～2 that was the ratio of exhale and inhale of breathe. All the operations were very good. There were not any unexpected things and complications.The breathe difficuty was lysisedafter 2～3mins of operation. The laryngismus and the temporary breathe stop that werecaused by stimulus of the tracheobronchoscopy were quickly lysised.In the operations ofesophegus foreign body the stifle and lacking oxygen did not happen. This paper showed:HFJV could improve the state of lacking oxygen and lighten stifle and ensure supplying oxy-gen and reduce the complications of operations and raise the safty factor of operations.
