未来海战中医院船重伤员72小时直接护理项目及时数分析

摘　　要：目的通过对198例重症创伤伤员直接护理项目及时数分析,探讨未来海战中医院船上各类伤员护理特点,为培训内容、人力配置提供依据。方法护理项目及时数采用PRN分类方法,筛选198例各伤部重症伤员,对其入院后24、48、72 小时直接护理项目及频数进行统计与筛选,以此为依据运用随机服务系统理论中的M G K模型,对医院船护理人员配置进行排队系统研究。结果平时198例各伤部创伤伤员入院72 小时内所获得的直接护理项目按PRN分类为8类92项;筛选后为7类37项,保留项目占40.22%。其中,诊断、治疗、呼吸类项目护理比重大;第1个24小时内所获得的直接护理时数与第2、第3个24小时内所获得的护理时数在颅脑伤不具有统计学意义(P > 0.05),胸外伤、腹部伤、四肢伤、复合伤具有统计学意义(P <0.05)。模拟300床位医院船护理人员配置数量66 名(不含手术室护士)。结论医院船收治伤员在72小时内救治工作量大,应根据前三天的工作量,合理配置人力,筛选后的直接护理项目可作为参战护理人员培训的重点内容。Objective To observe the direct nursing time and categories of severe wounded patients received in 72 hours after the admission. Methods Use PRN system to calculate the direct nursing time and categories of 198 patients with severe trauma receiving between the first 24, 48, 72 hours after the admission. According the model of M/G/K to study the system of manpower distribution. Results The direct nursing category which the patients received during the first 72 hours is 8 kinds 92 terms, after screening the total number is 7 kinds 37 terms. The patient＇s number who received the particular direct nursing categories in treatments, diagnostic, procedures and respiration parts is much higher than the one in the other parts, The direct nursing time which the patients received between the first 24 hours and the second, the third 24 hours is not different in statistics in brain trauma （P 〉 0.05） ; but it＇s different in chest, abdomen, limb and compound trauma （P 〈 0.05）. Simulating 300 beds in hospital ship,the manpower distribution of nurses is 66 （except operation room nurse）. Conclusions The nursing workloads of patients with severe trauma in the first 72 hours are excessive. In training process we should attach importance to the direct nursing categories in treatments, diagnostic, procedures and respiration parts.
