摘 要:目的:探讨经冠状静脉窦逆行灌注心脏不停跳方法对体外循环术中心肌保护的效果。方法:16只健康杂种犬随机分为实验组(浅低温逆行灌注心脏不停跳组)与对照组(中度低温心脏停跳组),每组8只。体外循环(CPB)150min。于6个时点采静脉血,检测血浆肌钙蛋白I(cTnI)。结果:两组CPB期间cTnI与CPB前比较均有不同程度升高,实验组与对照组cTnI在CPB中各时点比较差异均有统计学意义(P<0.05)。结论:cTnI可作为CPB过程中检测心肌保护效果的敏感指标;心脏不停跳法有利于减少cTnI的释放,具有较佳的心肌保护效果。Objective:To study the myocardial protective effects on retrograde perfusion via coronary sinus in heating heart. Methods: 16 health mongrel dogs were randomly divided into 2 groups which were experimental group(with mild hypothermic retrograde perfusion via coronary sinus in heating heart n =8) and control group(on arrest heart with mide hypothermic). Cardiopulmonary hypass(CPB)was performed for 150 minutes, cTnⅠ of tow groups were respeptively measured and contrasted at 6 different time intervals. Result:cTnI in two groups increased in varying degrees during CPB. cTnⅠ in experimental group increased more slowly. The peak of cTnI appeared when CPB finished, cTnI in experimentel group were lower than the same time in control group (P 〈 0. 05). Conclusion: Increased plasma cTnⅠ might be sensitive detecting guidelines in myocardial protective effects. Beating heart were propitious to reduce the release of cTnⅠ. Myocardial protective effects in experimental group was superior than that of control group.
