摘  要:目的定量检测同种带瓣主、肺动脉(conduit valved homograft,CVH)移植后受体细胞替换供体细胞的比例,探讨其对CVH耐久性、免疫原性的作用。方法①建立液氮冻存和4℃保存犬CVH移植于同种异性受体犬腹主动脉之实验模型。②采用组织DNA提取、定量PCR技术检测CVH中供、受体细胞所占比例。③免疫组化法检测CVH中碱性成纤维细胞生长因子(FGF-2),结合透射电镜和扫描电镜观察,反映CVH移植后的组织活性。结果①CVH移植后存在受体细胞替换供体组织的过程,长入CVH的受体细胞主要是成纤维细胞及内皮细胞。②具有正常结构和功能的受体细胞长入CVH对于CVH术后长期结构和功能的保持具有重要作用。③受体细胞进行性地替换CVH细胞,使供体细胞在CVH组织中所占比例逐渐减少;加之受体内皮细胞长入并覆盖CVH内膜,起“封闭抗原”作用。结论犬CVH移植后受体细胞替换导致CVH的免疫原性下降、免疫性损伤减轻,从而延长其耐久性。Objective To study the role of ingrowth of host cells in durability and immunogenicity of conduit valved homograft(CVH) by quantitative analysis of the percentages of host cells and donor cells after CVH implantation. Methods ( 1 ) Canine CVH stored at 4 ℃ and CVH cryopreserved in liquid nitrogen were heterotopically transplanted in the abdominal aorta of the recipient canine. (2) Tissue DNA extraction and quantitative polymerase chain reaction(PCR) technique were performed on CVH to calculate the ratio of donor cells and host cells. (3) The tissue viability of the CVH after implantation was analyzed through detecting alkaline fibroblast growth factor (FGF-2) by immunohistochemical staining method and by observing tissues in transmission electron microscope and scanning electron microscope. Results There is a process of the ingrowth and replacement of host cells in the implanted CVH, and ingrowing host cells are mainly fibroblasts and endothelial cells. Conclusion ( 1 )It is very important for maintaining the long-term structures and functions of the implanted CVH that host cells with normal ultrastructures and functions grow into the CVH tissues. (2) Host cells continuously replace donor cells to lessen the ratio of the donor cells in the CVH tissues; host endothelial cells grow and cover the CVH intima to blockade CVH antigens, which can reduce the immunogenicity and the immune lesion of the CVH. Thus the durability of the CVH is prolonged.
