摘 要:目的:对不同肺功能和血氧分压的稳定期慢性阻塞性肺病患者进行Loewenstein认知量表调查,并以健康者为对照,试图发现稳定期慢性阻塞性肺病患者认知功能在不同肺功能和血氧分压间的差异。方法:选择2002-08/2004-06因慢性阻塞性肺疾病急性发作曾在北京军区总医院重症监护病房和呼吸内科住院治疗后转为慢性阻塞性肺疾病缓解期患者64例(患者组),男41例,女23例。①分组:按肺功能将患者分为肺功能较好组40例(第1秒用力呼气量占预测值百分比≥68.44%和第1秒用力呼气量占用力肺活量的百分比≥59.62%)和肺功能较差组24例(第1秒用力呼气量占预测值百分比<68.44%和第1秒用力呼气量占用力肺活量的百分比<59.62%)。按动脉血氧分压值将患者分为血氧分压偏高组27例犤≥(8.05±0.83)kPa犦和血氧分压偏低组37例犤<(8.05±0.83)kPa犦。选择同期本院门诊常年体检的健康人69名为对照组,男43例,女26例。两组对象均自愿参加。②评估:肺功能评估和卧位血气指标检测采用肺功能仪;认知功能评估采用Loewenstein认知量表(共分7大部分和27项分测验:定向力16分;视知觉16分;空间知觉12分;动作运用12分;视运动组织28分;思维操作28分;注意力及专注力4分;总分为7项分测验得出的分值总和。各大部分满分12~28分,分值越高代表认知功能佳)。结果:慢性阻塞性肺疾病患者64例,健康者69名,均进入结果分析。①患者组与对照组Loewenstein认知量表评定结果:患者组定向力、视知觉、空间知觉、动作运用、视运动组织、思维操作、注意力各项分测验评分及总分均明显低于对照组(t=1.981~3.409,P<0.05~0.01)。②不同肺功能患者Loewenstein认知量表评定结果:肺功能状态较差组患者定向力、视知觉、动作运用、视运动组织、思维操作、注意力各项分测验评分及总分均明显低于肺功能状态较好组患者(t=2.271~3.107,P<AIM: To evaluate eonvalescent patients with chronic obstructive pulmonary disease with different pulmonary function and partial pressure of hlood oxygen hy using the Loewenstein occupational therapy cognitive assessment, and try to find out the differences of cognitive function among convalescent patients with chronic obstructive pulmonary disease with different pulmonary function and partial pressure of blood oxygen taking healthy people as controls.METHODS: Sixty-four patients (41 males and 23 females) with chronic obstructive pulmonary disease at convalesecne (patient group), who had been treated in the Intensive Care Unit and Department of Respiratory Medicine from August 2002 to June 2004, were involved in this study. ① Grouping: According to pulmonary function, the patients were divided into better pulmonary function group (n =40, the percentage of forced expiratory volume in first second to the predicted value ≥ 68.44% and the percentage of forced expiratory volume in first second to forced vital capacity ≥ 59.62%) and worse pulmonary function group (n=24, the percentage of forced expiratory volume in first second to the predicted value 〈 68.44% and the percentage of forced expiratory volume in first second to forced vital capacity 〈 59.62%). According to arterial partial pressure of oxygen, the patients were divided into higher arterial partial pressure of oxygen group (≥ (8.05±0.83) kPa, n=27) and lower arterial partial pressure of oxygen group (〈 (8.05±0.83) kPa, n=37);Other 69 healthy physical examinees in this hospital at the same period were taken as controls (control group. 43 males and 26 females); All the subjects took part in this study voluntarily. ②Evaluation: The pulmonary function meter was used to assess pulmonary function and detect the blood gas indexes at clinostatism;Their cognitive function evaluated with the Loewenstein occupational therapy cognitive assessment (consisted of 7 parts and 27 suhtests: orientation 16 points
