摘  要:目的探讨极低出生体重儿(VLBW)生后骨代谢的特点及影响因素。方法总结55例出生体重≤1500g的VLBW出生后1个月内血钙(Ca)、磷(P)、碱性磷酸酶(ALP)变化的特点及长骨X线改变。结果VLBW出生时合并低钙血症19例(34.5%),孕周≤32周者18例(94.74%),母亲的血钙水平与婴儿的血钙呈正相关。VLBW出生时ALP增高者41例(74.5%),2周后低钙血症得到纠正,但ALP持续升高(P<0.001),3周后有40%VLBWX线显示骨质疏松的改变,3例发生骨折。结论VLBW出生时已有骨代谢酶的异常,动态监测ALP的变化可早期发现VLBW骨代谢病。Objective To explore the characteristics of the metabolism and the possible related factors in very low birth weight(VLBW) newborns. Methods 55 newborns with birth weight less than 1500g were measured for serum calcium (Ca), phosphorus (p), alkaline phosphatase (ALP) and bone X-ray were taken within one month. The data were analyzed retrospectively. Results 19 cases (34.5%) with hypocalcemia at birth, 18 cases (94.7%) at gestation age ≤32w.There was positive correlation between the serum calcium level of mother and infant when 41 cases (74.5%) had higher ALP, and the hypocalcemia was corrected after two weeks, however the ALP was persistently higher (P〈0.001), 40% VLBW had bone dense change after 3 weeks and 3 cases had fracture. Conclusions VLBW has abnormal bone metabolism at birth, monitoring serum ALP level change can detect early bone metabolism abnormality.
