背向散射技术与SPECT对急性前间壁心肌梗死的评价

摘　　要：目的:观察在急性前间壁心肌梗死中梗死区心肌超声背向散射参数变化,并研究其与单光子发射型计算机断层扫描(SPECT)所测相应心肌节段灌注缺损值的关系。方法:心肌梗死组20例,对照组15例,均于冠状动脉造影术后第4～5天行超声背向散射参数分析及SPECT。结果:与正常对照组比较,心肌梗死组梗死区心肌标化背向散射积分值明显增高,背向散射积分周期变化幅度(CVIB)减低,周期变化延迟时间校正值延长。梗死区心肌CVIB与SPECT相应节段心肌血流灌注缺损值呈负相关。结论:背向散射技术与SPECT在冠心病心肌缺血诊断中具良好的一致性,是一种无创评价心肌缺血的新方法。Objective： To observe the changes of ultrasonic integrated backscatter parameters in infarct myocardium, and study the relationship between the change in integrated backscatter and the myocardial perfusion defects of relative myocardial segments examined by single photon emission computed tomography （SPECT）. Methods： Twenty acute anteroseptal myocardial infarction patients and fifteen controls were received AD-IBS analysis and SPECT examination 4-5 days after coronary angiography. Results： The calibrated myocardial integrated backscatter increased, the cyclic variation in integrated backscatter （CVIB） decreased, and the delayed time of cyclic variation of integrated backscatter prolonged in myocardial infarct group compared with control ,group. CVIB of infarction was negatively related to myocardial perfusion defects of relative myocardial segments. Conclusion： Myocardial integrated backscatter is consistent with SPECT in the diagnosis of myocardial ischemia, and it is a new noninvasive method of myocardial ischemia assessment.
