重组组织因子途径抑制物对兔髂动脉粥样硬化新生内膜增生的抑制

摘  要:目的:探讨兔髂动脉球囊剥脱术后局部灌注重组组织因子途径抑制物蛋白对血管内膜增生的抑制作用。方法:实验于2003-02/2004-11在哈尔滨医科大学附属第二医院实验动物中心及心内科完成。选择雄性日本大耳白兔24只,随机分为2组,重组组织因子途径抑制物组12只和生理盐水组12只。①建立髂动脉球囊损伤模型:将3.0~3.5mmOTW球囊导管沿导丝送入左髂动脉,使球囊近端距腹主动脉远端1cm。以606kPa扩张球囊,反复推拉3次,以剥脱血管内膜。撤出导丝,经OTW球囊按分组分别注入重组组织因子途径抑制物蛋白300μg和生理盐水1.5mL。球囊压力50kPa,保留20min后撤除。术后给予动物高胆固醇饮食(胆固醇、蛋黄粉、脂肪)4周。②测定横切面管腔面积、内弹力板内管腔面积、外弹力板内管腔面积、计算新生内膜面积(内膜面积=内弹力板内管腔面积-横切面管腔面积)、中膜面积(中膜面积=外弹力板内管腔面积-内弹力板内管腔面积)和新生内膜与中膜面积比,采用组织病理学方法,利用计算机图像处理。每段血管的数值均由2名人员读取3张切片(间隔30μm)数值计算均值而得。观察局部血管组织病理学变化,观察实验血管段新生内膜增生情况。结果:2组24只动物均进入结果分析,实验过程中无脱失值。①造膜结果:经过2周高胆固醇饮食饲养后兔血清总胆固醇水平高于饲养前犤(17.73±7.42),(1.10±0.26)mmol/L,P<0.01犦,经形态学观察确认为典型的髂动脉粥样硬化损伤模型。②组织病理图像分析:重组组织因子途径抑制物组外弹力板内管腔面积、内弹力板内管腔面积、横切面管腔面积分别高于生理盐水组犤(1.59±0.41),(1.21±0.40),(0.80±0.22)mm2,(1.16±0.36),(0.79±0.47),(0.24±0.19)mm2,P<0.01犦。重组组织因子途径抑制物组新生内膜面积、新生内膜与中膜面积比显著少于生理盐水组犤(0.23±0.21)mm2,0.74±0.64,(0.55±0.24)mAIM: To investigate the inhibitory effect of local perfusion of recombinantt issue factor pathway inhibitor protein on intima hyperplasia in atherosclerotic arteries after balloon stripping in rabbits. METHODS: The experiment was carried out in the experimental animal room and Department of Cardiology, Second Affiliated Hospital, Harbin Medical University from February 2003 to November 2004. Twenty-four male Japanese big-ear white rabbits were randomly divided into two groups: recombinant tissue factor pathway inhibitor group (n=12) and saline group (n=12). ① Model establishment of iliac arterial balloon injury: A 3.0-3.5 mm balloon tube was put into left iliac artery via guide wire to make proximal balloon 1 cm away from distal abdominal aorta. The balloon was expanded under 606 kPa, pushed and pulled repetedly for 3 times to strip intima. The guide wire was pulled ou4 and then recombinant tissue factor pathway inhibitor (300 μg) and saline (1.5 mL) were infused via OTW balloon in the two groups respectively. The balloon pressure was 50 kPa and kept for 20 minutes. After the stripping, the rabbits were given diet of high cholesterol (cholesterol, yolk powder, fat) for 4 weeks. ②The luminal area of transverse section, internal elastic luminal area and external elastic luminal area were determined, the new intima area (intima area=internal elastic luminal area-luminal area of transverse section), media area (media area= external elastic luminal areaO-internal elastic luminal area), and the ratio of new intima area to media area were calculated, and then the data were analyzed with computer image by the histopathological method. The value of each vessel was the average value of 3 sections (with an interval of 30 μm) by. 2 personnels. The pathological changes of local vascular tissues and the new intima hyperplasia of experimental vascular segaments were observed.
