慢性再生障碍性贫血中医分型的相关因素分析

摘　　要：目的:探讨性别、发病年龄、病程、血细胞减少程度、既往药物治疗和机体免疫水平等因素与慢性再生障碍性贫血(CAA)中医辨证分型的关系。方法:采用回顾总结病史,并运用免疫比浊法和流式细胞仪技术检测37例肾阳虚证和23例肾阴虚证CAA患者血清补体C3、C4含量、免疫球蛋白(Ig)含量以及外周血(PB)T细胞亚群的表达水平,分析其与中医证型的关系。结果:①CAA肾阳虚和肾阴虚组患者的发病年龄、性别、病程、血细胞减少程度和既往用药均无差异。②两组血清C3、C4含量和T细胞亚群表达存在显著差异,前者C3、C4含量明显降低,后者CD8+细胞高表达且血清C3含量与PBCD8+细胞表达呈正相关(r=0·30),与CD4/CD8比值呈负相关(r=-0·31),P<0·05。两组Ig含量以及NK细胞表达均降低且无组间差异。结论:CAA中医辨证分型和免疫指标血清补体C3、C4含量和T细胞亚群CD4+、CD8+表达水平密切相关,但与患者的性别、发病年龄、病程、血细胞减少程度、既往用药无关。Abstract： Objective： To discuss the relationship between factors of sex, age, couse of disease, degree of hypocytosis, past treatments and immunoresponsive level in patients with chronic aplastic anemia and TCM differenciation of syndromes. Methods：Review and summerize the medical records, and use immune turbidimetry and flow cytometry to detect serum content of complement 3, 4 and immunoglobin（Ig）, and expression level of T subsets in peripheral blood （PB） in 37 patients of kidney- yang deficiency group and 23 patients of kidney- yin deficiency group with CAA, and to analysis their relationships. Results ： ① There is no difference between kidney- yang deficiency group and kidney - yin deficiency group in factors of sex, age, couse of disease, degree of hypocytosis, past treatments and immunoresponsive level of body. ②Serum content of 03, C4 and expression level of T subsets in PB are remarkably different between two groups. The former group shows lower level of C3, C4 in serum, the latter group shows high expression of CD8^＋ cells;and the serum content of C3 has positive correlation with expression of PBCD8^＋ cells and negtive correlation with value of CD4/CD8. Conclusion： Differenciation of syndromes of CAA has association with immunal indices of serum content of C3, C4 and expression level of peripheral CD4^＋ , CD8^＋ cells, no relation with factors of sex, age, couse of disease, degree of hypocytosis, past treatments.
