摘 要:目的:检验大鼠骨髓干细胞和脊髓神经干细胞移植对大鼠神经根脊髓内植入治疗臂丛神经根性撕脱伤的作用。方法:实验于2002-03/2004-04在哈尔滨医科大学附属第一医院动物室完成。成年雄性SD大鼠3只用于骨髓基质细胞提取;新生1~3d雄性SD大鼠5只用于脊髓细胞提取;成年雌性Wistar大鼠18只制作C5,C6,C7,C8,T1臂丛神经根性撕脱伤模型。骨髓基质干细胞及脊髓神经干细胞培养至第3代时标记5-溴-2-脱氧尿苷。将大鼠分为3组,每组6只。骨髓干细胞移植组和脊髓神经干细胞移植组分别于损伤脊髓内注入相应干细胞10μL(1×1011L-1),对照组损伤脊髓内注入10μL生理盐水。两个月后观察大鼠运动功能(采用grooming实验评价标准,分为0~5分,0分为严重障碍,5分为正常);电生理检查结果;大鼠颈髓行病理免疫组织化学检查结果。结果:纳入大鼠18只,实验过程骨髓干细胞移植组死亡2只、脊髓神经干细胞移植组死亡2只、对照组死亡3只。进入结果分析11只。①大鼠神经功能恢复:脊髓干细胞移植组1只大鼠左前肢恢复行走功能,左前爪已伸开,其他3只分别为4分,3分和1分;骨髓神经干细胞移植组3只达2分,1只无恢复;对照组3只存活大鼠无一只恢复。②大鼠电生理检查结果:电生理可见脊髓神经干细胞移植组大鼠肌电图有明显的收缩波。骨髓干细胞移植组大鼠肌电图可见肌肉纤颤波。③大鼠颈髓免疫组织化学结果:植入脊髓神经干细胞和骨髓干细胞均能存活,移植的脊髓神经干细胞分化为突触明显的神经细胞,而骨髓干细胞移植只有较少的细胞分化为突触较长的运动神经元。脊髓神经干细胞较骨髓干细胞分化好,游走广。结论:①脊髓神经干细胞和骨髓干细胞移植对臂丛神经根性撕脱伤神经根再植具有促进作用。②脊髓神经干细胞移植较骨髓干细胞移植对臂丛神经根性撕脱伤神经根再植的功�AIM: To compare the effects of transplantation of bone marrow stem cells and spinal stem cells into spinal nerve root on the brachial plexus avulsion in rats. METHODS: The experiment was done in the Animal Laboratory of the First Affiliated Hospital of Harbin Medical University from March 2002 through April 2004. Bone stromal cells were extracted from three male adult SD rats, and spinal cells were obtained from five male SD rats aged 1-3 days, and 18 female adult Wistar rats were selected to establish injured models of C5,C6,C7,C8,T1. Bone marrow stromal cells and spinal progenitor cells were cultured to the third passage and then were labeled by 5-Bromo-2-Deoxyuridine (BrdUrd). All the rats were divided into three groups with 6 in each group. Different stem cells (10μL, 1×10^11L^-1) were injected into the injured spinal cord in the two transplantation groups, and 10μL normal saline was injected into the spinal cord in the control group. Two months later, motor function was assess by using grooming experiment standard (scores 0-5:0 as severe disorder, 5 as normal). Electrophysiology examination, pathological examination and immunohistochemical examination were performed. RESULTS: Only 11 rats were involved in the result analysis, and the others were dead during the experiment (2 in the bone marrow stem cell group, 2 in the spinal neural stem cell group and 3 in the control group). ① Recovery of neurological function: In the spinal neural stem cell group, 1 rat recovered to walk (left forelimb), with extended left anterior claw. The scores on motor function were 4, 3 and 1 for the other 3 rats. In the bone marrow stem cell group, the scores on motor function were 2 in 2 rats, but there were no changes in one rat. In the control group, none of the three survived rats had changes in motor function. ② Electrophysiology examination: Electromyography showed obvious contractive wave in the spinal neural stem cell group, and fibrillation wave of muscle in the bone marrow stem cell
