摘  要:目的探讨煤工尘肺与T淋巴细胞及微量元素的关系。方法采用BAS(Biotin-Avidin system,生物素链霉亲和素系统)组织化学法检测煤工尘肺患者和正常对照组外周血T淋巴细胞亚群(CD3、CO4和CD8),FAAS(flame atomicabsorption spectrophotometry)法检测患者和正常对照组血清Cu、Zn、Fe和Mg含量。结果各期煤工尘肺患者和正常老年对照组相比较CD3的水平下降显著(P<0.01);早期(Ⅰ期、Ⅱ期)煤工尘肺患者CD4反应性增加(P<0.01);各期煤工尘肺患者和对照组相比较CD8的水平上升显著(P<0.01);CD4/CD8的水平均下降(P<0.01)。煤工尘肺患者与老年对照组比较外周血清Cu升高,差异有非常显著性(P<0.01),Cu/Zn比值升高差异有显著性(P<0.05)。结论煤工尘肺患者的细胞免疫力下降,并伴有免疫调节紊乱,与微量元素Cu、Zn的含量变化密切相关。Objective To investigate the relationship between coal worker's pneumoconiosis (CWP) and T-lymphocyte subtype and trace elements. Method T-lymphocyte subtypes, CD3, CD4 and CDs, were examined by biotin-avidin system (BAS) immunohistochemistry in patients with CWP and healthy controls in their peripheral blood. Their serum levels of copper (Cu), zinc (Zn), iron (Fe),calcium (Ca) and magnesium (Mg) were measured by flame atomic absorption spectrophotometry (FAAS). Result Number of CD3 declined significantly in patients with CWP at varied stages, and number of CD4 reactively increased in patients with CWP at stages Ⅰand Ⅱ, as compared to that in the elderly healthy controls (P〈0.01). Number of CDs increased and ratio of CD4 to CDs declined significantly in patients with CWP at varied stages, as compared to that in the elderly healthy controls (P〈0.01). Serum level of Cu and ratio of Cu to Zn increased significantly in patients with CWP, as compared to that in the elderly healthy controls (P〈0.01 and P〈0.05).Conclusion Cellular immune function declined in patients with CWP due to disturbance of immune regulation, which closely associated with changes in serum level of Cu and ratio of Cu to Zn. It provided a useful evidence for further studies on element medicine and therapy of CWP.
