血液透析对尿毒症患者胃肠运动功能障碍的影响

摘　　要：目的探讨血液透析对尿毒症患者胃肠运动功能障碍的影响.方法应用胃肠测压技术对26名尿毒症患者在进入透析前(尿毒症组)及充分透析4周后(治疗组)进行了消化间期移行性复合运动(MMC)检测,连续测定240min;应用放射免疫法检测了上述人员的空腹血浆胃动素水平;有8名尿毒症患者进行了电子胃镜检查.结果①尿毒症组MMCI期、Ⅱ期出现的比例明显高于治疗组13.3%,尿毒症组有MMCⅢ期者仅19.2%,明显低于治疗组77%,(x2=18.6,P＜0.01);②在有MMCⅢ期患者中,尿毒症组胃窦和十二指肠的收缩频率明显低于治疗组(t=3.3～12.4,P＜0.05,P＜0.01);③尿毒症组空腹血浆胃动素水平明显高于治疗组(t=19.6,P＜0.01);④8名行胃镜检查的尿毒症患者中,6人为慢性浅表性胃炎,2人为贫血性胃改变.结论尿毒症患者存在胃肠运动功能障碍,主要表现为MMC的异常和高胃动素血症.血液透析可改善尿毒症患者的胃肠运动功能障碍.Objective To study the effects of hemodialysis （HD） on disturbance of gastrointestinal movement function in patients with uremia.Methods The migrating motor complex （MMC） and motilin of 26 patients with uremia were measured before HD and after sufficient HD 4 week.Results ①MMCⅠ, Ⅱ of uremia patients were appeared higher frequency than that of the treat group （P〈0.01）, MMC Ⅲ of uremia patients were signifiearit lower than that of treat group （X^2 = 18.6, P 〈0.01）; ②the contractive frequency of antral and duodenal in uremia patients with MMC Ⅲ was significant lower than that of treat group （ t = 3.3 - 12.4, P 〈0.05, P 〈0.01）, limosis motilin of uremia patients were higher than that of treat group （ t = 19.6, P 〈 0.01）. Conduson Uremia patients have a disturbance of gastrointestinal movement function, and the essential manifestation is MMC abnonnity and high level motilin in plasma.The disturbance of gastrointestinal movement function were able to improved by HD.
