摘  要:目的在慢性肾脏病(CKD)不同分期比较体表面积标准化单血浆法与双血浆法99Tcm-DTPA血浆清除率(sGFR和tGFR)测定肾小球滤过率(GFR)的可靠性。方法入选患者男107例,女83例,平均年龄(50.3±4.9)岁。分别用单血浆法和双血浆法测定CKD患者99Tcm-DTPA血浆清除率,在CKD不同分期比较sGFR和tGFR的相关性和偏离程度,并建立直线回归方程。结果当tGFR≥30ml·min(?)·(1.73m2)-1时,sGFR与tGFR的平均绝对偏差百分数显著小于tGFR<30ml·min(?)1·(1.73m2)(?)1时(P<0.001),相关系数大于后一组;可建立直线回归方程:tGFR=-3.552+1.147×sGFR(r=0.973,P<0.001)。结论当tGFR<30ml·min-1·(1.73m2)-1时,sGFR显著偏离tGFR;当tGFR≥30ml·min-1·(1.73m2)-1时,sGFR与tGFR显著相关,可用sGFR估计tGFR。Objective To compare the glomerular filtration rate (GFR) by single-sample and twosample method with ^99Tc^m-DTPA in patients with chronic kidney disease (CKD) and to evaluate the reliability of single-sample method. Methods 190 CKD patients, 107 male, 83 female, averaged (50.3 ±4.9 ) years old. GFR was measured with both single-sample method and two-sample method. In different stages of CKD, correlation and bias were compared, also the linear regression equation was obtained. Results The average bias percentage of sGFR as well as tGFR in tGFR≥30 ml · min^-1· (1.73 m^2)^ -1 group was significantly less than in tGFR 〈 30 ml · min ^-1 · ( 1.73 m^2 )^ - 1 group( P 〈 0. 001 ) , correlative coefficient was greater than later group. Linear regression equation : tGFR = - 3. 552 + 1. 147 × sGFR ( r = 0. 973, P 〈 0. 001 ). Conclusions When tGFR 〈 30 ml · min^-1 · ( 1.73 m^2 )^ -1 , sGFR markedly deviate the tGFR, whereas tGFR ≥ 30 ml · min^-1· ( 1.73 m^2 )^ -1 , sGFR correlated significantly with tGFR. Single-sample method can also be used in tGFR≥30 ml · min^-1· (1.73 m^2)^ -1group.
