EFFECTS OF TPN AND INDOMETHACIN ON STRESS RESPONSE AND PROTEIN METABOLISM AFTER SURGERY

摘  要:比较了全肠外营养(TPN)和消炎痛对创伤性应激反应和蛋白质代谢的影响。将19例因贲门癌行经腹根治性全胃切除的中等创伤病人,分为对照组、全肠外营养(TPN)组和消炎痛组,监测了体温、血浆蛋白、应激激素、尿中尿素氮、肌酐和3-甲基组氨酸的变化。结果发现:TPN组病人血浆C-反应蛋白、儿茶酚胺、皮质醇、胰岛素均增高;消炎痛组病人体温降低,儿茶酚胺和皮质醇升高不明显,尿中尿素氮和3-甲基组氨酸排出减少。说明TPN能促进蛋白质合成,消炎痛能减轻应激反应,抑制蛋白质分解。The study was planned to evaluate effects of TPN and indomethacin on stress response af-ter trauma, and protein metabolism in patients who had received total gastrectomy for cardiac cancer ofstomach. 19 cases was divided into control ,TPN,and indomethacin groups. Body temperature,plasma pro-teins , stress hormones andurinarv excretion of BUN, Cr and3-MH were determined. The result showedthat plasma C-reactive protein, insulin and nitrogen retention were significantly increased in TPN group,and that body temperature,plasma levels of cortisol and catecholamine,and urinary excretion of UBN and3-MH were remarkably decreased in indomethacin group. It is conclued that TPN could promote proteinsynthesis and nitrogen balance,and that indomethacin might reduce stress response and protein catabolismIn traumatic patients
