血液透析对尿毒症患者IL-8血浆水平及基因表达的影响

摘　　要：目的了解血透及不同透析膜对尿毒症患者IL-8血浆水平及基因表达的影响。方法采用酶联免疫吸附试验(ELISA)测定IL-8血浆水平,逆转录多聚酶链反应(RT-PCR)检测基因表达。结果正常对照组、尿毒症非透析组及血透治疗组之间的IL-8血浆水平均有显著性差异,正常对照组与尿毒症非透析组的IL-8mRNA表达无显著性差异,血透组与其它二组间的IL-8mRNA表达有显著性差异。聚砜膜组的IL-8血浆水平及mRNA表达低于铜仿膜组及铜氨膜组,而铜仿膜组及铜氨膜组之间无显著差异。结论血透可引起患者IL-8血浆水平及mRNA表达的异常,再生纤维素膜(尤其是铜仿膜)导致IL-8血浆水平及基因表达改变的能力强于聚砜膜,测量IL-8血浆水平及基因表达有助于评价不同透析膜的生物相容性。Objective To investigate the Influence of the hemodialysis and the different dialysis membranse on the interleukin-8 plasma level and gene expression in the uremic patients. Method The plasma concentration of IL-8 was measured with enzyme-linked immunoabsorbent assay （ ELISA ）. The gene expression of IL-8 was measured with reverse transcription-polymerase chain reaction（RT-PCR）. Results There is a significant difference between the plasma IL-8 level of the ESRD patients on conservative therapy and that of the patients receiving hemodialysis. The ESRD patients on conservative therapy are not different to the healthy subjects in IL-8 mRNA expression. However, the IL-8 mRNA expression of the patients receiving hemodialysis is higher than that of the two control groups. The IL-8 plasma and mRNA level of the patients using the regenerated cellulose membrane （CU, CR） is higher than that of those using PS membrane. Conclusion Hemodialysis can lead to the abnormal IL-8 plasma and mRNA level of the uremic patients, it is more serious when the cellulose membrane （especially CU） is used. The measurement of IL-8 plasma level and mRNA expression in PBMC is useful to evaluate the biocompatibility of dialysis membranes.
