综合疗法治疗上皮型中晚期卵巢癌(附21例临床报告)

摘　　要：目的为了探讨综合疗法对上皮型中晚期卵巢癌的疗效、免疫功能及生存期的影响。方法21例上皮型中晚期卵巢癌患者采用术前2～3个疗程的TP方案的化疗,同时给予3～4次的腹腔热化疗,无腹水者采用气腹针穿刺,灌注温盐水1000～1500ml(40～42℃)+DDP50～60mg;有腹水者留置深静脉导管,待腹水排净后灌注温盐水1000～1500ml(40～42℃)+DDP50～60mg,3～4周后行减瘤术,术后再行3～4个疗程的TP方案辅助化疗,治疗期间口服补中益气、扶正固本、滋阴养血中药综合治疗。结果有效率95.2%(20/21),1年生存率88.9%(16/18),3年生存率66.7%(10/15),CA125明显下降,与治疗前相比有显著差异(P<0.01),同时机体细胞免疫功得到明显提高。副反应主要为骨髓抑制。结论综合疗法能明显提高上皮型中晚期卵巢癌治疗的总有效率,延长生存期。Objective To study the ettects ot combined therapy on immune function and survival tor advanced epithelium ovarian carcinoma. Methods Twenty-one patients with advanced epithelium ovarian carcinoma underwent preoperative 2- 3 cycle chemotherapy （TP schema） with 2-3 cycle celiac thermochemotherapy. The patients without ascites were punctured with insufflation needle,and then infused with warm saline of 1 000-1 500 ml（40-42 ℃ ） plus cisplatin,while the patients with ascites using a deep vein pipe were infused with warm saline of 1 000-1 500 ml （40 - 42 ℃ ） plus cisplatin of 50 - 60 mg after completely removing ascites. These patients underwent assistant chemotherapy （TP） of 3 - 4 weeks＇ cycles after operation, at the same time underwent combined therapy with Chinese medicines. Results Total effective rate was 95.2%, 1-year survival rate was 88.9 % （16/18） and 3-year survival rate was 66.7 % （10/15）. CA125 significantly decreased and cellular immunity significantly enhanced compared with pretreatment（P〈 0.01）. The side effect was mostly marrow suppression. Conclusion This therapy can apparently increase the total efficiency for treating advanced epithelium ovarian and prolong the length of survival.
