摘  要:采用短肠大鼠全肠外营养模型,动物随机分为四个组.静脉营养或喂养连续8天后,取大鼠的胰和肝标本,应用光镜、电镜和图像分析观察胰和肝的组织学和超微结构的变化,结果显示:2%谷氨酰胺—全肠外营养组胰腺泡截面积较常规全肠外营养组明显增大,腺泡细胞内酶原颗粒含量明显多于常规全肠外营养组.常规全肠外营养组肝组织在中央静脉周围均有脂肪颗粒沉积,而2%谷氨酰胺—全肠外营养组仅有少数呈轻度的脂肪颗粒沉积.电镜观察示:常规全肠外营养组肝细胞内有大量脂滴存在,2%谷氨酰胺—全肠外营养组肝细胞内粗面内质网、核糖体和糖原的含量明显多于常规全肠外营养组,饮食组胰和肝组织的组织结构和超微结构均正常.研究提示:含2%谷氨酰胺的静脉营养液可防止胰外分泌部的萎缩和防止肝脂肪变性.饮食刺激可维持胰和肝结构和功能的完整.The short bowel model parenterally fed rats were used in this study. The rats were divided into four groups. After they were given intravenous infusion or chow-fed continuouslly for 8 days, the samples of the pancreas and the liver wre taken and histologically sliced. The morphological changes of the pancreas and the liver were examined under light microscope and TEM, and measured by using Image Analysis System. The results were as follows: The cross -sectional areas of the pancreatic acinar cells in the GLN group increased significantly compared with these in the STD group. The amount of acinar cells containing zymogen granules in the GLN group was more than that in the STD group under TEM. Histological examination of the liver revealed extensive centrilobular fatty vacuoliation in the STD group, but no fatty droplets were found in the hep tic cells in the STD group. The rough endoplasmic reticulum, nucleoglucse body, and the glycogen in hepatic cells in the GLN group were significantly more than those in the STD group under TEM. The study demonstrated that 2% glutamine-enriched parentedal nutrition could prelect the pancreas against atrophy and prevent fatty liver after massive small intestine resection.
