超声引导下治疗钙化性冈上肌腱炎的初步研究

摘　　要：目的探讨超声引导下治疗钙化性冈上肌腱炎的临床价值.方法超声引导下采用16号针对16例钙化性冈上肌腱炎的钙化灶进行反复穿刺治疗,治疗后第2、3、6、12和24周进行随访,记录症状缓解情况和声像图变化.应用视觉模拟法(VAS)、美国肩肘关节外科医师协会评价系统(ASES)和Constant评分方法对比治疗前后的变化.结果治疗后患者自觉症状改善的占 81.2%.治疗24周以后,钙化灶消失8例,缩小5例,钙化灶平均消失时间是(6.1±0.5)周.治疗后的 3～24 周的VAS评分结果与治疗前的差异均具有显著性意义(P＜0.05).治疗后6～24周ASES评分与治疗前差异均具有显著性意义(P＜0.05).治疗前后Constant评分改变无显著性意义(P＞0.05).结论超声引导下治疗钙化性冈上肌腱炎是一种有效的方法,绝大多数患者的症状能得到显著缓解.Objective To study the clinical values of needling under ultrasonography guidance in calcified supraspinatus tendonitis. Methods Sixteen patients with supraspinatus calcific tendonitis received needling therapy under uhrasonography guidance. Two, three, six, twelve and twenty-four weeks after treatment, the patients were evaluated using a visual analog scale（VAS） for pain, the ASES（association of shoulder and elbow surgeons） self-questionnaire, Constant＇ s score, and uhrasonography, Results Improvements in the symptoms were noted in 81.2% of patients. Sonograms were changed in 81.3% of patients after 24 weeks （with complete clearance of the calcific deposits in 50%）. Improvements in the VAS and ASES scores were noted after 3 weeks and 6 weeks respectively. No improvements in the Constant scores were noted after treatment. Coaclusions By the methods of treatment under ultrasonography guidance for calcified supraspinatus tendonitis, symptoms and disability are relieved. The method is effective and should be further studied clinically.
