抗生素支气管肺泡灌洗对重症老年细菌性肺炎患者肺功能及炎症介质的影响

摘　　要：目的探讨抗生素肺泡灌洗对老年细菌性肺炎患者肺功能及炎症介质的影响,寻找治疗新途径。方法将175例老年细菌性肺炎患者分成2组,其中灌洗组90例,采用抗生素肺泡灌洗联合抗生素静脉给药;非灌洗组85例单纯抗生素静脉给药;测定治疗前、中、后的肺功能和炎症介质。结果灌洗组较非灌组痊愈快,疗程短,肺功能(FEV1%、PEF、MVV%)显著改善(P<0·05);炎症介质内毒素(KEV)、肿瘤坏死因子-2(TNF-2),白细胞介素-6(IL-6)、脂质过氧化物(LPO)显著降低(P<0·01)。结论抗生素肺泡灌洗联合静脉给药治疗重症老年细菌性肺炎,肺功能及炎症介质恢复至正常水平较快,疗程较短,可减少炎症对肺功能的损害,具有保护肺功能的远期效果,是治疗老年细菌性肺炎比较理想的途径。Objective To study the effect of anitibiotic bronchoalveolar lavage on pulmonary function and inflammatory mediator of the patients with sevre senile bacterial pneumonia, and quit a new way of treatment. Methods 175 patients with senile bacterial pneumonia were divided into two groups： experiment group （90 patients, antibiotic broncboalveolar lavage and antibiotic intravenous infusion ） and control group （86 patients, antibiotic intravenous infusion）. Pulmonary function and inflammatory mediator were determined before, during and after treatment respectively. Results Compared experiment group with control group, the patients recovered more quickly, the duration of treatment was shorter, the pulmonary function was improved significantly（ p〈 0.05）. KEV, TNF-2, IL- 6, LPO decreased significantly（ p 〈 0.01）. Conclusion The patients with severe senile bacterial pneumonia who is treated by antibiotic bronchoalveolar lavage and antibiotic intravenous infusion can recover more quickly. Conclusion The patients with severe senile bacterial pneumonia who were treated by antibiotic bronchoalveolar lavage and antibiotic intravenous infusion can recover more quickly in the pulmonary function and the inflammatory mediator, the duration of treatment is short tened. The damage of inflammation to the pulmonary function can be decreased. It is a good way of treatment.
