摘  要:[目的]了解噪声作业工人扩展高频听力状况,探讨扩展高频测听对早期发现噪声性聋的敏感性.[方法]应用频率范围为0.5~20 kHz的纯音听力计对588名五金工业噪声作业工人的听力进行检测,以180名健康年轻人作为对照.[结果]在99.05dB(A)的噪声强度下,当0.5~6 kHz的阈值未出现异常时,10~18kHz的阈值明显高于对照组(10~18 kHz,P<0.01),对最大输出没有反应耳数的比率明显高于对照组(14~18 kHz,P<0.01).不同工龄组比较,6~10年工龄组的10~18 kHz的阈值明显升高.[结论]高强度噪声引起的听力损失,10~20 kHz听阈的变化早于0.5~6 kHz.工龄影响10~20 kHz听阈,其对最大输出引不出反应和听阈阈值的变化可作为早期噪声性听力损失及噪声易感者的指标.扩展高频测听对早期发现噪声性聋具有一定的帮助.[Objective]To study the auditory function with extended high frequency for workers with noise exposure history and evaluate the sensitivity of diagnosing noise induced hearing loss with this method. [Methods]Five hundred eighty-eight workers with noise exposure history were examined by pure tone audiometry(0. 125-8 kHz). One hundred and twenty normal subjects served as the control group. [Results]The noise levels at working sites ranged from 99.05dB(A). Compared with the control group, noise exposed subjects showed minor changes in the thresholds from 0.5 to 6 kHz , whereas the thresholds were elevated in the high frequency range of 10- 16 kHz ( P 〈0. 001), The incidence of ears without any response to the maximum output at the frequency range of 14 to 20 kHz was much higher ( P 〈 0. 01) than that of the control group. The threshold of 10 to 18 kHz increased with increase of working time(6- 10years ). [Conclusion]In noise exposed ears, the occurrence of threshold changes in the high frequency range(10 to 20 kHz ) is earlier than that in the low frequency range (0.5 to 6 kHz). Working time also affects the 10-16 kHz hearing thresholds. The lack of responses to maximum output and the changes in thresholds in the high frequency range (10 to 20 kHz) can be used as indices for the diagnosis of early noise induced hearing loss and for the assessment of the susceptibility of the subjectnoise damage. The extended high frequency audiometry is thought helpful to screen noise induced hearing loss early.
