摘 要:目的探讨重症急性胰腺炎(severe acute pancreatitis,SAP)大鼠肺脏诱导型一氧化氮合酶(iNOS)mRNA的表达及其与肺组织损伤的关系和丹参的治疗效应。方法45只W istar大鼠随机分为3组。SAP模型组、丹参组均以5%牛磺胆酸钠经胰管内逆行注射(1 m l/kg)复制SAP模型,丹参组则在制模后立即肌肉注射丹参(2 m l.kg-1.d-1),每6 h 1次,共给药4次。对照组仅行假手术。各组动物在术后24 h测血一氧化氮(NO)、淀粉酶(AMY)和腹水量。使用原位杂交和图像分析检测肺iNOS mRNA的表达强度,并观察其病理变化。结果AP模型组血中NO、AMY、腹水量和肺脏iNOS mRNA表达显著高于丹参组和对照组(P<0.05,P<0.01);丹参组病理改变较模型组减轻。结论SAP时肺组织iNOS mRNA高表达导致NO过度生成并与肺脏损伤有关;丹参能抑制肺组织iNOS mRNA的过度表达,从而对肺组织起保护作用。Objective To investigate the expression of inducible nitric oxide synthase (iNOS) mRNA in lungs and the effect of salvia miltiorrhiza on lung injury in rats with severe acute pancreatitis (SAP). Methods A total of 45 Wistar rats were randomly divided into 3 groups: SAP model group (SP, n = 15 ), salvia mildorrhiza group ( SM, n = 15 ) and sham operation group ( SO, n = 15 ). Severe acute pancreatitis was induced by intraductal injection of 5% sodium taurocholate ( 1.0 ml/kg) in SP group and SM group. Sham operation was only made in SO group. Salvia mihiorrhiza (2 ml · kg^-1· d^-1) was intramuscularly injected immediately after SAP was induced in SM group, and the same dose was administrated every 6 h. The same volume of physiological saline was administrated in SP group and SO group. The volume of ascites and serum nitric oxide (NO) and amylase (AMY) were determined, iNOS mRNA expression in lung were examined by in situ hybridization. The pathologic changes of lung was observed after 24 h of rat model establishment. Results The volume of ascites and serum levels of AMY and NO and the expression of iNOS mRNA in lung in SP group were higher than in SM group and SO group ( P 〈 0. 05, P 〈 0. 01 ). The pathologic changes of lung in SM group were less than in SP group. Conclusion Over-expression of iNOS mRNA in lung plays an important role in lung injury in SAP rat models and salvia miltiorrhiza inhibits the expression of iNOS mRNA and ameliorate the lung injury.
