血清C反应蛋白与高血压病心功能改变的关系及临床意义

摘　　要：目的探讨血清C反应蛋白(C-reactive prote in,CRP)含量变化与高血压病心功能改变的关系,并分析其临床意义。方法研究对象来源于2002-08-2005-01在本院门诊及住院确诊为原发性高血压病的患者。依据美国纽约心脏病学会(NYHA)1928年提出的心功能分级方案,选择高血压病无心功能障碍、高血压病轻度心功能障碍及高血压病重度心功能障碍的患者各26例,应用TurboxR特定蛋白分析系统(芬兰)以散射比浊法测定血清CRP含量,并评定其心功能,分别在患者确诊后24 h内进行测定。选取同期正常健康人26例作为对照。结果正常对照组、高血压病无心功能障碍组、高血压病轻度心功能障碍组和高血压病重度心功能障碍组间年龄、病程、性别均无显著差异。正常对照组所有患者心功能评分正常,其血清CRP含量处于较低水平。高血压无心功能障碍组心功能评分正常,与正常组相同,其血清CRP含量与正常对照组相比无显著性差异(P>0.05)。高血压病轻度心功能障碍组心功能评分增高,与正常组相比有显著差异(P<0.05),其血清CRP含量与正常对照组及高血压轻度心衰组相比均有明显增高(P<0.01)。高血压病重度心功能障碍组心功能评分明显增高,与高血压轻度心功能障碍组相比有显著差异(P<0.01),其血清CRP含量与正常对照组、高血压无或轻度心功能障碍组相比均有明显增高(P<0.05)。结论血清CRP水平与高血压病心功能障碍的发生和程度密切相关。高血压病心功能障碍的预防和治疗应包括早期抗炎治疗。Objective To detect the serum level of C-reactive protein in the hypertension patients and analyze its relationship with the heart function impairment. Methods The hypertension patients who had no, or mild, or severe heart function impairment were selected and accordingly divided into 3 groups （n = 26 in each group）. Meanwhile another 26 healthy subjects served as normal controls. Tile heart function of all subjects were evaluated and scored according to heart function classified schedule published by New York Heart Disease Association （ 1928 ） and CRP levels were measured by scatter turbidimetry within 24 h after diagnosis. Results The scores of heart function for healthy patients and the hypertension patients without heart function impairment were within the normal range and their serum CRP levels were at the basic level. The scores and the serum CRP levels of the hypertension patients with mild heart function impairment were higher than that of healthy subjects and the hypertension patients without heart function impairment （P 〈 0.05）. The scores and serum CRP levels of the hypertension patients with severe heart function impairment were much higher than that of the other three groups （ all P 〈 0.05 ）. Conclusion The serum CRP levels in the hypertension patients has a close relationship with the development of heart function impairment. The prevention and cure of heart function impairment in hypertension patients should include anti-inflammatory therapy at the early stage.
