摘  要:目的:探讨多层螺旋CT曲面重建(curvedmulti-planarreconstructions,CMPR)在输尿管扩张性疾病的临床应用价值。方法:回顾分析患侧肾功能严重受损的输尿管扩张性疾病33例,手术前均行超声(USG)、静脉尿路造影(IVU)和多层螺旋CT增强检查,多层螺旋CT检查于静脉注射对比剂2~5min后行薄层容积扫描,经独立工作站后处理获得患侧输尿管的CMPR图像。结果:USG发现33例均患侧输尿管积水,但显示梗阻部位及病变不理想;IVU示患侧输尿管未显影;CMPR图像均清楚地显示了输尿管的扩张程度、梗阻部位、病变形态及其与周围组织之间的毗邻关系。结论:多层螺旋CT曲面重建在评价已有患侧肾功能严重受损、IVU检查输尿管未显影的输尿管扩张性疾病上具有重要的诊断价值。Objective: To investigate the clinical value of multi-slice spiral CT with curved multiplanar reconstructions (CMPR) in the expansionary ureteral diseases. Methods: Thirty-three patients with uretal expansion and serious renal failure in the same side were analyzed retrospectively. All the patients underwent USG, IVU and multi-slice spiral CT enhanced scanning before operation. The thin multi-slice spiral CT volume scan was completed 2-5 minutes after enhanced contrast was injected in vein. Images were postprocessed and the CMPR images of the viewed ureters were performed on the independent workstation. Resuits: By USG, the expansionary urinary tracts were detected and the lesions were not clearly displayed; by IVU, the urinary tracts were not filled by contrast; by CMPR, the dilated extent of ureters, the positions and features of the lesions and the relations with adjacent tissues of lesions were clearly displayed. Conclusion: CMPR plays an especially important role in the diseases cannot be displayed by IVU. diagnosis of serious renal failure with expansionary ureteral diseases cannot be displayed by IVU.
