血清NSE、CK-MB、尿β_2-MG测定对新生儿窒息后治疗的意义

摘　　要：目的探讨新生儿窒息后血清磷酸烯醇化酶(NSE)、肌酸激酶同工酶(CK-MB)、尿β2-微球蛋白(尿β2-MG)变化及其意义。方法按1minAgpar评分标准将108例足月窒息新生儿分为轻度窒息组(4～7分)和重度窒息组(0～3分),测定其生后48～72h、10～12d血清NSE、CK-MB、尿β2-MG并与40例足月正常新生儿对比。结果生后48～72h,重度窒息组和轻度窒息组新生儿血清NSE、CK-MB、尿β2-MG较为对照组显著增加(P<0.01),重度窒息组NSE、CK-MB、尿β2-MG也较轻度窒息组显著增加(P<0.01),生后10～12d重度窒息组新生儿血清NSE、CK-MB、尿β2-MG仍较对照组、轻度窒息组显著增加(P<0.01),轻度窒息组与对照组比较,差异无显著性(P>0.05);重度窒息组和轻度窒息组新生儿生后10～12d血清NSE、CK-MB、尿β2-MG均较生后48～72h明显下降,差异有显著性(P<0.01),对照组变化不明显(P<0.05)。结论窒息后新生儿脑、心、肾功能均有损伤,轻度窒息儿经治疗后脑、心、肾功能能迅速恢复,重度窒息儿虽经治疗,脑、心、肾功能也不能迅速恢复。Objective To explore the variations and mcanings of serum NSE, CK-MB and urnary β2-MG in full-term neonates with asphyxia. Methods 108 full-term asphyxiated neonates were divided into the severe group （0～3） and the mild group according to Apgar scores at I minute . 40 full-term normal neonates were selected the control group. Serum Neuron-specific enolas （NSE）, CK-MB, and uriary β2-microglobulin （β2-MG） were measured in the three group at 48～72 hours and 10～72 hours and 10～12 days after birth. Results NSE, CK-MB and β2-MG in the severe grounp and the mild group were highet than those in the control group at 48～72 hours after birth （P〈0.01）. Those in the severe group were higher than in the mild group,too （P〈0.01）. But NSE,CK-MB and β2-MG in the severe group were higher than those in the control group and the mild groupat 10～12 days after birth （P〈0.01）. However those in the severe group and the mild group were lower at 10～12days after birth at 48-72 hours. Conclusion Asphyxiated neonate fumctions of brian , and renal were damdages. The mild asphyxiated neonate functions of brian , heart, and renal were recovered puickly after the treatment. But the severe asphyxiated nrona asphyxiated neonate fumctions of brian , heart, and renal were not recovered quickly after the treatment.
