摘 要:目的通过兔髂动脉再狭窄模型评价和脉合剂防治动脉再狭窄的安全性和有效性。方法40只新西兰大白兔行右髂动脉再狭窄造模,术后随机分高脂组、和脉合剂组进行喂养。应用下肢动脉造影、电镜和病理检查,观察术后7、14、28、56、84d病变动脉的狭窄情况。免疫组化法检测增殖细胞核抗原(PCNA)和TUNEL的表达。结果影像学和病理学检查提示再狭窄手术造模成功。新生内膜和中膜以血管平滑肌细胞(VSMCs)为主,随术后时间的延长,VSMCs减少而泡沫细胞增多;细胞成分减少,胶原成分增多;新生内膜比例增大,中膜比例减小,以上变化高脂组较和脉合剂组更显著。与高脂组比较,和脉合剂组内膜和中膜PCNA阳性表达下降,TUNEL阳性表达增加。结论和脉合剂通过抑制血管壁细胞增殖和促进凋亡,安全、有效地防治动脉再狭窄。Objective To evaluate the safety and efficacy of a Chinese traditional medicine preparation Hemai Heji on the prevention and treatment of restenosis in rabbit iliac arterial re-stenotic models. Methods Right iliac arterial re-stenotic lesions were established in 40 New Zealand white rabbits, which were randomly divided into hypercholesterol diet group and Hemal Heji group post-operation. Diseased arteries were studied by means of arteriography, electroscopic and pathological methods from 7 d to 84 d after operation, in 5 periods of time. The expression of PCNA and TUNEL were detected with EnVision. Results Arteriographic and pathological studies revealed the successfully established re-stenotic models. Vascular smooth muscle cells (VSMCs) were predominant in the neo-intima and media. With the prolongation of post-operative duration, the number of VSMCs decreased while those of foam cells increased ; cellular composition decreased while collagen increased ; the ratio of neo-intima in the vessel wall increased while that of media decreased. Alterations were more significant in hyper-cholesterol diet group than those in Hemai Heji group. Compared to hyper-cholesterol diet group, the expression of PCNA in both neo-intima and media was decreased,and TUNEL was increused in Hemal Heji group. Conclusion The prevention and treatment of Hemal Heji on arterial restenosis are safe and effective. The effect might be achieved by inhibiting proliferation and enhancing apoptosis in the vascular cells.
