临床甲状腺功能检查新策略的医学决策学方法初步探讨

摘　　要：报告了三个医院八个观察组，６２３例血ｓ－ＴＳＨ、ＦＴ＿４、ＦＴ＿３分析结果，以医学决策学方法计算其单项检测、两项联检、三项联检的敏感性、特异性、阳性和阴性似然比、机遇外一致率Ｋ值，ｓ－ＴＳＨ可用于甲状腺疾病的初筛试验，三者联检效益更高，只费用更大，作者认为在我国目前情况下，临床上似仍以ＴＴ＿４、ＴＴ＿３与ｓ－ＴＳＨ联检为佳。The results of the serum s-TSH,FT_4 ,FT_3 levels of 623 cases were analysed in eight groups of three hos-pitals.The sensitivity, specificity,positive and negative likelihood ratio,agreement beyond chances K,thatvalue computed by s-TSH,FT_4,FT_3 tests were determined in single,both-or three union examination withmedical decision making. The s-TSH can be used to primary screen test for thyroid disease. The combinationof s-TSH,FT_4,FT_3 test was more effective, but cost a lot.So far the combination of TT_4,TT_3 and s-TSHtest are best one at clinic in Chna.
