检测血吸虫循环抗原的多价单克隆抗体诊断试剂盒的研制和应用

摘　　要：用杂交瘤技术生产抗血吸虫肠相关循环阳极抗原（ＣＡＡ）单克隆抗体与血吸虫卵糖蛋白单克隆抗体经混合后标记辣根过氧化物酶（ＨＲＰ），制成Ｄｏｔ－ＥＬＩＳＡ诊断试剂盒，检测轻、中、重不同疫区粪检阳性的血吸虫病人血清中循环抗原，阳性率分别为８４．３％、８７．２％和９１．５％。累计阳性率为８９．２％（６１９／６９４）。对健康人、肝吸虫、肺吸虫病人血清均未出现阳性反应，显示有较高的敏感性和特异性。ＥＰＧ＞８０的病人循环抗原检出率高干ＥＰＧ／８０的病人。说明循环抗原检出率与感染度有关。血吸虫病人经吡喹酮治疗后半年和１年循环抗原转阴率分别为７１．４％和８８．６％。结果表明．该试剂盒可用于确诊病人和考核疗效。试剂盒质量稳定，操作简便快速，整个试验可在１小时内完成，且不需特殊仪器，适于现场大规模查病应用。Polyvalent monoclonal antibodies kit has been developed by using 3B10G8 and 3B11E3 McAbs against schistosome gut associated anodic antigen and 3G4A4 McAb against egg glycoprotein after being mixed and then labelled with HRP.Circulating antigens of sera from 694 patients infected with Schistosoma japonicaum in hypo-, meso-and hyper-endemic areas were detected by the Dot-ELISA. The positive rates were 84.3%, 87.2% and 91. 5% respectively, but no positive were observed in 133 normal individuals, 51 patients with clonorchiasis and 20 with pargonimiasis. The level of circutating antigens in group with EPG>80 was found to be higher than that in group with EPG<80.The circulating antigens in 71.4% and 88.6% cases turned into negative 6 or 12 months after effective praziquantel therapy respectively.The positive rates of the maculating antigens detevted by enzyme conjugates of polyvalent monoclonal antibodies was higher than that of monovalent one. Variation determination of the kit in single batch and between batches showed that the variation coefficients were less than 10%. This method has advantages of simplicity and rapidity without special instruments needed.The kit can be used for diagnosis and treatment evalution in field trials.
