下壁心肌梗塞合并左前半阻滞的心电向量图诊断

摘　　要：对15例IMI合并LAH的患者进行心电向量图分析,并与心电图比较。结果表明,心电向量图诊断IMI合并LAH诊断阳性率明显高于心电图(P<0.05)。IMI合并LAH在心电向量图上有其特征性改变:额面QRS环初始0.02～0.03s向量向上,呈顺钟向运行,0.04s后呈逆钟向运行,最大QRS环移向左上象限,QRS大部分环体位于X轴之上。Vectorcardiographic features of 15 patients with inferior myocardial infarction (TMI) complicated .with left anterior hemiblock(LAH) and the comparison between vectorcardiograrn and electrocardiogram were analysed. Results showed that the reliability of vectorcardiograni for diagnosing IMI with LAH was higher than that of electrocardiogram significantly (P<0. 05). The characteristics of vectorcardiogram of IMI with LAH were QRS loop in frontal plane was superior direction and clockwise in initial 0. 02~0. 03 second, but counterclockwise rotation after 0. 04 second, the maximed QRS loop deviated left superior area,major area of QRS loop located above X axis.
