矽肺患者肺功能与血气分析

摘　　要：对６７名Ⅱ、Ⅲ期矽肺患者做了肺功能（ＶＣ，ＦＶＣ，ＦＥＶ_１，ＦＥＶ_１／ＦＶＣ，ＭＶＶ，ＤＬＣＯ和血气分析（ＰａＯ_２，ＰａＣｏｇｍ_２，ｐＨ）。结果表明Ⅲ期矽肺肺功能较Ⅱ期矽肺降低，其中ＶＣ，ＦＶＣ，ＭＶＶ降低有显著差异，ＰａＯ_２和ｐＨ随矽肺期别增加呈降低有显著差异，ＰａＯ_２降低尤为明显，ＰａＣＯ_２呈增高趋势。肺功能与ＰａＯ_２改变显著相关，与ＰａＣｏ_２和ＰＨ无相关关系。ＰａＯ_２＜１０ｋＰａ发生率与尘肺劳动力鉴定肺功能损害评价结果一致。ＰａＯ_２＜１０ｋＰａ可以作为评价尘肺患者劳动能力的有价值的指标。The lung funcfion(including VC , FVC,FEV1,FEV1/FVC, MVV,DLCO,etc.)and the arterial blood gas analysis including PaO2,pH,etc.)of 67 patients with silicosis at Ⅱand Ⅲstage were studied.The results showed the lung function of patients with silicosis at Ⅲ stage was obviously lower than that of patients with silicosis at Ⅱ stage. In particular,VC,FVC,MVV were obviously decreased,PaO2 and pH were decreased with increas of the stage of silicosis, but PaO2 had tendency increased with it.There were a close correlations between degree of lung function impaired and level of PaCO2 decreased,but did not show significantly correlation with PaCO2 and pH. In view of the fact that the frequency of PaO2<10kPa was consistent with degree of lung functional impairment,so that PaO2<10kPa mag serve a valuable index for evaluation of disability in patients with silicosis.
