三尖瓣下移畸形超声心动图特征研究

摘　　要：３４例Ｅｂｓｔｅｉｎ畸形患者详细的超声心动图研究发现Ｅｂｓｔｅｉｎ畸形的三尖瓣病变除了瓣叶起始部位下移和发育不良外，瓣叶末端附着异常也是Ｅｂｓｔｅｉｎ畸形重要的病理学特征。瓣叶末端异常附着，呈线性排列附着到右室壁的肌脊上，引起瓣叶活动受限；同时发现Ｅｂｓｔｅｉｎ畸形患者病理解剖差异较大，尤其是本研究发现单纯的后瓣下移和前瓣细小的不典型病例，因此超声心动图是提供Ｅｂｓｔｅｉｎ畸形详细的、完整的形态学特征的重要手段。A detailed study of the two-echocardiographic examination revealed that the anatomical distal attachment of the leaflet is the important anatomical feature in addition to the downward displacement and dys plasia of tricuspid valve in 34 patients with Ebstein’s anomaly.The anomalous distal leaflet attachment in a linear fashion to a muscular ridge of the right ventricular prohibits normal motion of the leaflet.We also found the wide variability of anatomic features in these patients especially the isolated displacement of the posterior leaflet and the small anterior leaflet which differs from the typpical Ebstein’s anomaly.
