克山病胰岛损害的病理图像定量分析

摘　　要：应用免疫组织化学与图像分析技术相结合的方法,研究了62例克山病患者尸检材料胰岛的病理变化。结果表明克山病患者胰岛存在多项结构参数的异常,主要表现为:(1)胰腺组织切片内胰岛素样免疫反应产物面积比例(Ins^+/panc.％)下降,P<0.002;(2)胰岛素免疫反应产物面积与胰岛截面积比例(Ins^+/Isl,％)明显缩小,P<0.001;(3)胰岛平均胰岛素免疫反应产物面积(Ins^+area)显著减少;(4)胰岛截面数密度(N、)降低(P<0.05);(5)胰岛形状因子数值明显升高(SF),P<0.002。图像分析结果所发现的多项胰岛参数的变化进一步表明,克山病胰岛损害的严重性与复杂性,提示胰岛损害可能对克山病心肌病变的发生发展起一定作用。In this paper the pathologic features on the islets lesions of the patients with Keshan Disease (KD) were studied by immunohistochemical and quantitative analysis methods. The results demonstrated that there were lesions found morphologically in pancreatic islets of the patients with KD, including atrophy, reduction of number, abnormal arrangement and degeneration of B cells in the islets. In the quantitative analysis, the ratio of insulin immune active product area to pancreatic tissue area as well as the ratio of insulin immune active product to islet area obviously reduced while the shaper factor increased. The results suggested islets lesions in KD decreased the production and secretion of insulin, which may affect the myocardial energy metabolism. Therefore, the islets lesions are not only the results but also a pathologenic link in the occurrence and development of myocardial ' necrosis in KD.
