高血压性心脏病患者血浆内皮素的改变及卡托普利对其影响

摘　　要：以正常人２０例（对照组）和无左心室肥厚的原发性高血压患者２０例（ＥＨ组）作为对照，观察高血压伴左心室肥厚组３２例（ＬＶＨ组）的血浆内皮素变化；对ＬＶＨ组２８例患者行卡托普利治疗，观察治疗前后血浆内皮素的变化，ＬＶＨ组血浆内皮素含量（２０９．０７±６９．１２ｐｇ／ｍｌ）明显高于ＥＨ组（１６１．２１±４１．５２ｐｇ／ｍｌ）和对照组（１３４．６１±３４．８８ｐｇ／ｍｌ）（Ｐ均＜０．０１），且血浆内皮素含量和ＬＶＨ程度及血压水平呈正相关。卡托普利治疗后血浆内皮素含量明显降低（Ｐ＜０．０１）。本文结果表明内皮素可能参与高血压和高血压性ＬＶＨ的发生。：Ｐ＜０．０１．ｖｓｃｏｎｔｒｏｌ：△：Ｐ＜０．０５，△△：Ｐ＜０．０１，ｖｓＥＨｇｒｏｕｐ．２血压水平与血浆ＥＴ含量的关系（Ｔａｂ２）Ｔａｂ２ＴｈｅｒｅｌａｔｉｏｎｓｈｉｐｂｅｔｗｅｅｎｔｈｅｌｅｖｅｌｓｏｆｂｌｏｏｄｐｒｅｓｓｕｒｅａｎｄｔｈｅｐｌａｓｍａＥＴｃｏｎｔｅｎｔｓ：Ｐ＜０．０１．ｖｓｃｏｎｔｒｏｌ；△：Ｐ＜０．０５．ｖｓｍｉｌｄｈｙｐｅｒｔｅｎｓｉｏｎｇｒｏｕｐ；△△：Ｐ＜０．０１，ｖｓｍｏｄｅｒａｔｅｈｙｐｅｒｔｅｎｓｉｏｎｇｒｏｕｐ．ＥＨ和ＬＶＨ组血浆Ｅ?The plasma endothelin in 32 patients with hypertensive left ventricular hypertrophy (LVH group)were measured compared with normotensive subjects(n=20)and essential hypertensive patients without LVH(n=20)(EH group).Thethe changes in plasma endothelin before and after captopril treatment in LVH group(n=28)was also investigated.The results showed that the levels of plasma endothelin in LVH group were significantly higher than those in EH group(P<0.01)and normotensive subjects(P<0.01),with the levels of plasma endothelin being significantly higher in the patients with severe hypertension. A positive correlation between the plasma endothelin and blood pressure and the degrees of LVH was found.The levels of plasma endothelin were significantly decreased after captopril treatment. The results suggest that endothelin may play an important role in the pathogenesis of hypertension and hypertensive LVH.
