北京地区708例带器宫外孕分层相对危险分析

摘　　要：对北京地区1990年期间经手术、病理或腹腔镜检查确诊的1420例全部新发宫外孕病例中的708例带器宫外孕者,根据IUDs类型分层对宫外孕与IUDs类型之间的关系进行了相对危险分析。结果表明,带器宫外孕的发生取决于IUDs预防宫内孕的效果,并且与其类型密切相关,该结论通过对IUDs类型的分层相对危险分析得以证实。分层分析表明,各种IUDs对宫外孕的发生有不同的影响,因此妇女选择和使用节育效率和安全性高的IUDs对降低宫外孕发生的危险非常重要。研究结果进一步显示,在非妊娠对照中,仅TCu对宫外孕发生有明显的保护作用(其ORmh为0.44,P<0.05);在妊娠对照中,各种IUDs的ORmh均大于1,表明如果使用失败,各种IUDs均增加宫外孕发生的危险。即使对宫外孕有明显保护作用的TCu一旦使用失败,其保护作用也随之降低。All new ectopic pregnancy cases ( 1420 ) in Beijing area were obtained through operation, pathologic and laparo-scope diagnosis technique. Among them,' 708 cases were using IUDs. Stratified relative risk analysis method was used to explore the relationship between ectopic pregnancy and using IUDs. The degree of preventing ectopic pregnancy is determined by using the IUDs effectiveness of preventing intrauterine pregnancy. The contraceptive effectiveness of IUDs is associated with the type of IUDs. It is confirmed by stratified relative risk analysis in this study. The result of stratified analysis method shows that different IUDs has different effect on ectopic pregnancy. This is necessary for selecting safer IUD and improving the skill of using IUDs. The analysis results also show that in non-pregnancy group, the ORmh of TCu is 0. 44 (P< 0. 05 ) , implying its significant protective effect on avoiding ectopic pregnancy. Otherwise, in pregnancy control group,various IUDs including TCu IUD (ORmh >1,P< 0. 01) .increase the risk of ectopic pregnancy significantly. This implies that the function of avoiding ectopic pregnancy of TCu IUD disappears when use failure occurred.
