经皮球囊二尖瓣扩张术的疗效研究

摘　　要：1988年8月～1993年5月完成经皮球囊二尖瓣扩张术(PBMV)115例,其中94.5%(111例)操作成功。观察证明,PBMV 可使二尖瓣口面积增加一倍左右。明显的改善血液动力学,心功能改善1～2级。对获得双侧交界裂开的病人,其疗效与直视二尖瓣交界分离术相似,如包括不同程度交界裂开的病人在内,则其总体效果与闭式二尖瓣交界分离术相同。从而证明了 PBMV 是治疗风湿性二尖瓣狭窄的有效方法。One hundred and fifteen patients with rheumatic mi- tral stenosis were treated with Percutaneous Balloon Mi- tral Valvuloplasty (PBMV) from August,1988 to May, 1993.110 patient (94.5%) had successful operations. The observations showed that PBMV can enlarge the stenostic mitral orifice area over 1 time (from 1.04±0. 26cm to 2.08±0.32 cm,P<0.001) and improve the patient's hemodynamies (immediately after PBMV,there was an decrease in the LAP from 3.37±1.17 Kpa to 1. 65±0.84 kPa,P<0.001,MVPG from 2.34±0.94 kPa to 0.46±0.45 kPa,P<0.001) and cardiac function 1 to 2 grade.PBMV can achieve the same therapeutic ef- fect as that of open mistral valvotomy if the patient's dou- ble fusion commissures are opened by balloon dilatation. If the patients are included whose fusion commissures are opened in different degrees,the whole effect is the same as that of CMV.The study demonstrated that PBMV is an effective method for treating most patients with rheumatic mitral stenosis.
