外伤性截瘫病人双下肢甲襞微循环观察

摘　　要：通过对128例外伤性截瘫病人的甲襞微循环观察,主要表现有微血管管袢轮廓不清,血色呈暗紫色,细动脉、细静脉明显痉挛变细,畸形管袢明显增多。管径粗细不均,管袢内红细胞聚集明显,血流缓慢,多为粒缓流、粒流、管袢周围明显渗出。总加权积分为9.23±1.03。其结果提示,外伤性截瘫病人均有严重微循环障碍。因此,定期进行微循环检查,对监测病情,指导治疗,延长病人生存期是十分必要的。Nailfold microcirculation of both lower limbs was observed in 128 patients with traumatic paraplegia. The result showed that the main changes of the nailfold microcirculation included obscurity, disarrangement , enlargement and distortion of capillary loops, spastic stenosis of arterioles and venules, increase in abnormal loops, dark purple in microcirculatory blood, aggregation of erythrocytes, slow-up of blood flow (mostly slow granular flow or granular flow) and exudation aground the loops. The sum of scores was 9.23±1.03. It suggests that the patients with traumatic paraplegia have severe disturbances of microcirculation. Therefore, it is very necessary to determine the mictocirculation periodically, not only for monitoring patient's conditions and directing treatment, but also for providing a basis for rehabilitation of the traumatic paraplegia patients.
