带臀上血管深上支髂骨膜瓣治疗髋关节骨性关节炎的应用解剖

摘　　要：在１７具（３４侧）成尸标本上，对臀上动脉深上支及髋骨膜血供的有关应用解剖学进行了研究。该动脉主干自梨状肌上孔发出后，在臀中肌深面沿臀小肌起始部上缘呈弧形向前，至髋前上棘与旋髋深动脉和旋股外侧动脉开支吻合。主于在髋嵴中点下方６．７±０．６（６～８）ｃｍ。沿途发出４～７支骨膜支呈扇形分布髋骨翼。其起始处外径３．１±０．５（２．２～４．２）ｍｍ，主干长９．８１０．９（７．５～１２）ｃｍ，终末外径１．２±０．３（０．８～１．７）ｍｍ：起始处距股骨颈中点５．５±０．５（４．０～７．８）ｃｍ。伴行静脉均为双支，汇入处外径４．４±１．０（２．２～５．５）ｍｍ。内膜瓣截取面积为５ｃｍ×７ｃｍ。he blood supply to ilium periosteum flap,the deep division of the superior gluteal artery,was studied on 34 cases of lower limb specimens.The superior gluteal artery divides into superfa-cial divsion and deep division at superior aperture of pyriform. The deep division is further devidedinto superior and inferior branches running between the gluteal medius and minimus muscles.Small branches(4～7)arising from the deep division,which bear the length of 9.8 cm and exter-nal diameters of 3.1 mm to 1.2mm,spread to the periosteum at the wing of ilium. A periostealflap in the size of 5 cm×7 em based on the deep division of the superior gluteal artery could betransferred to the hip joint for the treatment of osseous arthritis.
