摘 要:幽门螺杆菌与胃炎、消化性溃疡有着非常密切的关系,但诊断方法仍以胃镜采样然后进行培养涂片、尿素酶试验为主;或使用需要一定设备的ELISA方法进行检测,难以在一般有消化系统症状的患者中使用,给H.p感染的诊断和治疗带来了困难。本文对不需特殊设备的IEST用培养涂片和IELISA方法进行了比较和应用。我们应用免疫酶染色法(IEST)对90名作胃镜检查者血清中抗幽门螺杆菌(H.p)抗体IgG进行了检测。其H,p感染的检出率为74.4%,高于细菌培养和镜检的检出率。灵敏度为94.9%,特异度为64.5%。各项指标均优于IELISA方法。在IEST抑制试验中,幽门螺杆菌与大肠杆菌空肠弯曲等无明显交叉反应,显示了其特异性,同时该法简便,经济,不需特殊仪器且易被受检者接受,值得推广应用。Pyloric helicobacteria are assooiated with gastritis,duodenal ulcer,and gastric ulcer. We developed a specific and sensitive method IEST(immuno-enzyme staining technique)that could detect serum immunoglobulin G antibodies. In a blind fashion we tested sera from 90 individuals and found that the positive rate(74.4%)was higher than the positive rate of the tissue culture(65.6%),tissue microscopy test(37.8% and IELISA(71.1%). According to the standard of the tissue culture and microscopy test, the sensitivity of IEST was94.9%and the specificity 64.5%. The results was all better than IELISA,Because there were false-negative result in tissue culture and microscopy test and in this study we did not find that Pvloric helicobacteria had immune crossreaction with other bacterium,it showed that the real specificify should be higher than 64.5%and the IEST was valuable to be applied as a diagnostic method of Pyloric helicobacteria infection.
