摘  要:采用血小板膜表面抗人活化血小板α颗粒膜蛋白(GMP-140)单克隆抗体SZ-51,以放射免疫法,检测24例急性心肌梗塞(AMI)患者发病1周内的血小板膜表面GMP-140、血浆凝血烷B_2(血栓素B_2,TXB_2)和6-酮-前列腺素F_(1α)(6-keto-PGF_(1α))的动态变化,并与20例健康人对照。AMI的GMP-140、TXB_2和6-keto-PGF_(1α)在发病早期已明显增高,48h达高峰,以后呈逐渐下降的规律性变化,这表明AMI早期,血小板高度活化。TXB_2和6-keto-PGF_(1α)与GMP-140正相关。梗塞范围广、病情重时,这三项指标增高更为显著。这提示,检测上述指标对监护病情,以及评估预后可能有辅助参考价值,并为发病早期选用抗血小板活性药物提供了客观依据。Dynamic changes of alpha-granule membrane protein (GMP-140) on the surface of platelet, plasma thromboxane B2 (TXB2) and 6-keto-prostaglandin F1α(6-keto-PGF1α) during the first week after onset of acute myocardial infarction(AMI) were studied in 24cases and 20 normal subjects by monoclonal antibodyradioimmunoassay.The results showed that the levels of GMP-140,TXB2 and 6-keto-PGF1α quickly increased after the onset of AMI, reached maximum in 48 hours,then reduced gradually.It indicates that the platelet in AMI patients was highly activated. There was positive correlation between the levels of GMP-140 and TXB2,also between GMP-140 and 6-keto-PGF1α,The levels of GMP-140, TXB2, and 6-keto-PGF1α markedly increased in extensive myocardial infarction cases, severe cases,and dead cases.The results indicate that determinations of GMP-140,TXB2 and 6-keto-PGF1α are helpful in monitoring AMI, evaluating the prognosis,and guiding antiplatelet therapy during the early stage of AMI.
