早期低剂量腹膜透析对改善尿毒症患者生活质量的价值

摘　　要：目的探讨早期低剂量腹膜透析治疗尿毒症患者的疗效。方法选择2007年3月-2010年2月慢性肾功能不全患者38例,分为两组:腹透组(n=19):接受常规药物治疗,并予间歇性腹膜透析,每日4000～6000ml,每周4～5d;对照组(n=19):接受常规药物治疗。随访24周。所有患者在治疗前、后采用症状频率和严重指数、SF-36生活质量量表对临床症状、生活质量进行评价,并监测血常规、肝肾功能、血糖、电解质及尿量。结果腹透组血红蛋白、血肌酐均较对照组明显改善。腹透组患者临床症状评分治疗前为11.80±5.41,治疗后第8周和第12周时分别降低至7.15±3.15(P<0.05)和5.94±3.70(P<0.01),对照组在治疗前、治疗后第8周和第12周时评分分别为11.23±5.35、9.80±4.02和10.11±4.48。两组间的差异持续至随访期结束。腹透组患者生活质量明显改善。结论早期低剂量腹膜透析可有效改善尿毒症患者临床症状和生活质量。Objective To investigate the efficacy of early low-dosage peritoneal dialysis (PD) in uremic patients.Methods From March,2007 to February,2010,38 uremic patients were assigned to two groups.PD group (n=19) received PD at dosage of 4,000-6,000 ml/day in 4 or 5 days per week besides routine treatments.Control group (n=19) received routine treatment as well as health education about uremia and guidance on diets.All the patients were followed up for 24 weeks.The clinical outcomes were evaluated by severity index of symptoms of uremia,and SF-36 tables for quality of life before and during the follow-up period,respectively.Volume of urine and routine experimental assay including hematoglobin,creatinine,glucose and electrolyte were monitored regularly.Results In the PD group,increased hemoglobin and decreased creatinine were found.The severity index of symptoms of uremia in the PD group decreased from 11.80±5.41 at the baseline down to 7.15±3.15 (P<0.05) at week 8, and 5.94±3.70 (P<0.01) at week 12.In the control group,the counterparts were 11.23±5.35,9.80±4.02 and 10.11±4.48,respectively.The significantly statistic differences persisted during the follow-up period.The quality of life in the PD Group was improved significantly.Conclusion Early low-dosage PD in uremic patients reveals efficacy in reducing severity index of uremic symptoms and improving quality of life.
