儿童过敏性紫癜肾损害54例相关因素分析

摘　　要：目的探讨儿童过敏性紫癜肾损害的相关因素。方法对2008年6月—2009年6月本院儿科收住的54例过敏性紫癜患儿临床表现及肾损害相关因素进行回顾性分析。结果 54例过敏性紫癜患儿首发症状主要为单纯紫癜者11例(20.37%),肾损害发生于紫癜复发者(76.92%)的比例高于初发者(60.71%)。表现为单纯紫癜、紫癜加关节症状、紫癜加消化道症状、紫癜加消化道加关节症状者肾损害发生率分别为45.45%、54.55%、75%、85%。结论过敏性紫癜复发患儿肾损害发生率较高;早期出现较多肾外症状尤其是消化道症状者,易发生肾损害。Objective To study anaphylactoid purpura(AP) in children and correlated factors of renal damage.Methods Fifty-four children of AP were retrospectively studied,i.e.the patterns of onset,clinical manifestations,and correlated factors of renal damage were analyzed.Results Of the 54 cases,there were only 11(20.37%) with an initial symptom of purpura simplex.The incidence of renal damage was higher in the recrudesce patients(79.52%) than in the first onset(60.71%).The incidence of renal damage were 45.45%,54.55%,75% and 85% for the patients with simple purpura,with purpura and joint symptoms,with purpura and gastrointestinal tract symptoms,and with all the above symptoms,respectively.Conclusion The incidence of renal damage is high in recrudescent AP children.And those with extrarenal symptoms during early phase,especially with gastrointestinal symptoms,tend to have renal damage.
