HBV逆转录酶区N236T突变与A181T突变患者临床特征的比较

摘　　要：目的了解HBV逆转录酶区N236T突变患者与逆转录酶区A181T突变患者临床特点的异同。方法对发生HBV逆转录酶区N236T单独突变与逆转录酶区A181T单独突变的患者体内HBsAg、HBV DNA和ALT水平以及HBV基因分型进行统计学分析。结果逆转录酶区N236T单独突变组与逆转录酶区A181T单独突变组相比较,后者会造成HBsAg水平的下降(P=0.02),但对血清HBV DNA(P=0.76)和ALT(P=0.40)无显著影响;此外,B型发生rtN236T单独突变的比例较高(77.78%vs.20.53%),而C型发生rtA181T单独突变的比例较高(79.47%vs.22.22%)。结论 HBV逆转录酶区N236T单独突变组与逆转录酶区A181T单独突变组体内HBsAg水平和HBV基因型构成存在明显差异。Objective To explore the clinical characteristics between patients with only N236T mutation and patients with only A181T mutation in the HBV reverse transcriptase(rt) region.Methods Cases of patients with only rtN236T mutation and patients with only rtA181T mutation were selected.Then the levels of HBsAg、HBV DNA and ALT and the HBV genotypes were analyzed with statistic Methods.Results Compared to the only rtN236T mutant,the only rtA181T mutant could lead to the decline of HBsAg(P = 0.02),but it had no effect on HBV DNA(P = 0.76)and ALT level(P = 0.40).Moreover,compared to each other,HBV B genotype was prone to generate only rtN236T mutation(77.78% vs.20.53%),while HBV C genotype was prone to generate only rtA181T mutation(79.47% vs.22.22%).Conclusion The HBsAg levels and the constituent of HBV genotypes between the group of only rtN236T mutant and the group of only rtA181T mutant varied significantly.
