摘  要:目的探讨原发性高血压(EH)患者动态血压脉压指数(PPI)与心功能及肾功能的关系。方法 EH患者178例,按动态血压PPI分为3组:PPI≤0.40组(64例),PPI0.41~0.50组(56例),PPI≥0.51组(58例)。3组患者均化验血肌酐,行24h动态血压监测及超声心动图检查。测量并计算24h平均收缩压(24hMSBP)、24h平均舒张压(24hMDBP)、24h平均脉压(24hMPP)、PPI、左室射血分数(LVEF)及左房室瓣血流E/A比值。结果 3组患者在左心室收缩功能、左心室舒张功能及肾功能受损方面差异均有统计学意义(P<0.01)。PPI≥0.51组心功能及肾功能受损的比例明显高于PPI≤0.40组。结论 EH患者PPI增大与心功能及肾功能受损密切相关,动态血压PPI可作为EH患者心功能及肾功能损害的预测因子。Objective The aim of the study was to investigate the relationship between pulse pressure index (PPI) of ambulatory blood pressure and function of cardiac and kidney in patients with essential hypertension (EH).Methods 178 patients with EH were divided,according to PPI,into group A (PPI≤0.40,n=64),B (PPI 0.41~0.50,n=56) and C (PPI≥0.51,n=58).The tests of serum creatinine(Cr),24-hour ambulatory blood pressure monitoring and echocardiography were performed.24-hour mean systolic blood pressure (24h MSBP),24-hour mean diastolic blood pressure (24h MDBP),24-mean pulse pressure (24h MPP),PPI,left ventricular ejection fraction (LVEF) and mitral valve flow E/A ratio were detected and calculated.Results The differences of left ventricular systolic and diastolic dysfunction and renal dysfunction among three groups had obviously statistical significance(P<0.01).The impairment proportion of the function of cardiac and kidney of PPI≥0.51 group was higher than it of PPI≤0.40 group.Conclusion Elevated PPI,closely correlated with the severity of function of cardiac and kidney,can be used as a predictor in patients with EH.
