心得安试验对儿童β受体功能亢进症的诊断价值

摘　　要：目的观察心得安试验对儿童β受体功能亢进症(BRHS)的诊断价值。方法分别描记21例BRHS儿童在卧位、立位及心得安试验时的12导联心电图,对比分析ST-T波振幅及形态变化。结果由卧位变为立位时,多个导联直立T波振幅降低、T波由直立或低平变为倒置、或倒置的T波振幅增加;21例患儿心得安试验均为阳性,心得安试验时T波改变较体位试验更为明显。主要表现为与卧位基础心电图相比,原有低平或倒置的T波变为直立,主要见于Ⅱ、Ⅲ、aVF导联。结论心得安试验可作为辅助诊断BRHS的有效方法。Objective To explore the diagnostic value of propanolol test on children with beta adrenoceptor hyperfunction syndrome.Methods 12-lead electrocardiogram ( 12 ECG) under supine,erect position and post-propanolol test were traced in 21 children with beta adrenoceptor hyperfunction syndrome.The morphous variations of T wave and ST segment were analyzed.Results T wave descended in lead Ⅰ、V3、V4、V5、V6 under erect position compared with supine position,and T wave inversion occurred in lead V1、V2 and deepened in lead Ⅱ、 Ⅲ、aVF、V1、 V2.ST segment amplitude had no significant change between under supine position and erect position.Compared with supine position,the electrocardiographic multi-lead erect T wave amplitude reduced,or erect、flat T wave turned to inversion,or inverted T wave amplitude deepened in erect position.Propanolol test were positive in all the 21 patients.On Propanolol test,T wave changes were more evident than the postural test,mainly changes were,compared with the supine position,the original flat or inverted T wave becomes upright,and were mainly seen in Ⅱ、 Ⅲ、aVF lead.Conclusion Propanolol test is an effective method in auxiliary diagnosis of beta adrenoceptor hyperfunction syndrome.
